A Method for Temporary Arterial Occlusion During Intracranial Aneurysm Surgery
نویسنده
چکیده
Introduction: The strategic use of temporary intracranial arterial occlusion is an important technique in aneurysm surgery. Metal clips similar to permanent aneurysm clips are employed but they are not ideally suited to the task. Methods: Balloon occluders placed around intracranial arteries at the Circle of Willis can provide immediate control of intra-luminal flow. These remotely controlled devices can be positioned in advantageous locations before they are needed and deployed if the situation arises. Results: The feasibility of balloon occluders was demonstrated in a patient with an anterior communicating artery aneurysm. A balloon occluder can be positioned without excessive dissection and permits the operating surgeon to proceed expeditiously and in a less crowded field. Conclusions: Hydraulically activated vascular occluders may offer a degree of technical refinement to the performance of temporary arterial occlusion.

Effect of nitrous oxide use on long-term neurologic and neuropsychological outcome in patients who received temporary proximal artery occlusion during cerebral aneurysm clipping surgery.
BACKGROUND The authors explored the relationship between nitrous oxide use and neurologic and neuropsychological outcome in a population of patients likely to experience intraoperative cerebral ischemia: those who had temporary cerebral arterial occlusion during aneurysm clipping surgery. METHODS A post hoc analysis of a subset of the data from the Intraoperative Hypothermia for Aneurysm Surg...
متن کاملSymptomatic and silent ischemia associated with microsurgical clipping of intracranial aneurysms: evaluation with diffusion-weighted MRI.
BACKGROUND AND PURPOSE Silent ischemic events are known to occur during diagnostic and interventional endovascular procedures between 10% and 69% of the time. The occurrence of silent and symptomatic ischemic events in the surgically treated population is not known, although atherosclerotic changes of intracranial vessels or within the aneurysms wall or neck area are seen often during surgery. ...
متن کاملEffect of Pre/Postconditioning at Temporary Clipping.
Intracranial aneurysms and their treatment is one of the leading problems of neurosurgery that create high mortality and morbidity. The technique of safe clipping is as generally used depends on the temporary occlusion of the cerebral vasculature during surgery. However, there is no exact data about temporary clipping or timing of this procedure. Preconditioning by exposure to sublethal hypoxic...

متن کاملA Surgical Model of Permanent and Transient Middle Cerebral Artery Stroke in the Sheep
BACKGROUND Animal models are essential to study the pathophysiological changes associated with focal occlusive stroke and to investigate novel therapies. Currently used rodent models have yielded little clinical success, however large animal models may provide a more suitable alternative to improve clinical translation. We sought to develop a model of acute proximal middle cerebral artery (MCA)...
